Consent Order Form

I would like to know where I can find a consent order form or is this just a letter that I can draft to stop child support but also state visitation changes.....

You can use a memorandum of judgment form.

Thanks for your response.

Is this a form that I can file without an attorney?

Is it as simple as going to the court and filing after the other party signs?

Does it need to be notarized?

Will we have to go in front of a judge?
