1. Under the phrase NORTH CAROLINA, fill in the name of the
county you are filing in.
2. At the beginning of the statement, before the phrase, being first
duly sworn, fill in your name (the Plaintiff).
3. After the statement, sign your name in the presence of a notary. (Remember,
Notary Publics can be found at banks and other financial institutions. Notary
Publics are also listed in the Yellow Pages of the phone book under Notaries
Public.) Also, this form must be signed before a Notary before taking the
forms to file with your Clerk of Court.
4. The Notary will fill in the remaining blanks on the Verification.
5. You will need (1) one original signed copy of this form and
(2) two copies of the signed copy of this form
6. Staple the original copy of the VERIFICATION to the back of the original copy
of the COMPLAINT. Staple the other copies of the VERIFICATION to the back
of the other copies of the COMPLAINT.
