1. Under the phrase, STATE OF NORTH CAROLINA, fill in the name
of the county you are filing in.
2. The Plaintiff and Defendant lines on the JUDGMENT will look the same
as the Plaintiff and Defendant lines on the COMPLAINT. Above the word,
Plaintiff, on the left side, fill in you name. Above the word, Defendant, on
the left side, fill in your spouse's name.
3. Do not fill in the date at the bottom of the form. The Judge will fill in
the date and will then sign the form.
4. You will need (1) one original and (2) two copies of this form.
