1. Fill in the name of the County at the top of the card.
2. Fill in the correct names (as they appear on the COMPLAINT)
of the Plaintiff (you) and the Defendant (your spouse).
3. The NOTICE OF ASSIGNMENT CARD will be mailed back to you
(the Plaintiff) to notify you of the trial date and location and whether
or not the CIVIL SUMMONS was served on your spouse (the Defendant).
Therefore, your (the Plaintiff's) mailing address, must be written in the center
of the back of the NOTICE OF ASSIGNMENT CARD. You must also place
a stamp in the upper right corner of the card.
